There are no drugs which are selectively operating on cerebral circulation now. At the same time a number of the medicines having the general spasmolytic activity, causing vasodilatation and improving blood circulation in various bodies and fabrics can positively influence to a degree blood supply of a brain, increase providing it with oxygen, strengthen its metabolic processes.
Expand brain vessels (mainly venules) organic nitrates and nitrites.
Less sharp, but longer vasodilatation of a brain (without selective effect on capacitive vessels) is caused by purine alkaloids (caffeine, theobromine) and their derivatives.
Niacin and drugs containing it (Xantinoli nicotinas, pikamilon, etc.) expand, rather quickly, brain vessels. Spasmolytic drugs (Nospanum and in adequate doses a papaverine, Dibazolum, etc.) are also capable to lower a tone of vessels of a brain and to strengthen cerebral circulation.
At spastic conditions of vessels of a brain, including at spastic forms of migraine, the dihydrogenated derivatives of alkaloids of an ergot are recommended (see Digidroergotamin, Dihydroergotoxin).
From drugs of new groups as the means improving cerebral circulation antagonists of calcium ions apply (see Verapamil). The cinnarizine which is considered as one of the most effective modern cerebrovascular remedies is (as it is established recently) an active blocker of membrane and calcium channels. The new antagonist of calcium ions nimodipin (see) is considered as especially active cerebral vazodilatator. Moderate expansion of brain vessels causes Dipiridamolum (see).
Improve blood supply and metabolic processes of a brain piperidic acid (see Aminalonum) and also piracetam and other nootropic drugs.
It is natural that at atherosclerotic defeats of vessels of a brain, anti-sclerous drugs (have to render positive effect see. Hypolipidemic means), and in thromboses of vessels of a brain - thrombolytic means (fibrinolysis activators, anticoagulants, antiagregant).
In extra hospital practice, at rather easy forms of disturbances of cerebral circulation, widely use some combined drugs containing a papaverine, Nospanum, caffeine, Dibazolum, niacin, etc. (see Nicoverinum, Nicospanum, Papazolum, Theodibaverinum, etc.). Rather widely appoint also pentoksifillin, or trental (see).
Now carry cinnarizine, Cavintonum to rather selectively operating cerebrovascular drugs and also nimodipin.
