Sciatica is the morbid condition which is followed by pains and disturbances of sensitivity of roots of a spinal cord. Sometimes disturbances of sensitivity are followed by peripheral paresis.
The combined damage of spinal nerves and their roots is called a radiculoneuritis. If covers of a spinal cord also are involved in pathological process, then pathological process develops as a meningoradiculitis.
Diseases of roots of a spinal cord are also designated as spondilogenny damage, or a spondilogenny radiculopathy (radikuloishemiya). When involving in process of a spinal node the radiculoganglionitis is diagnosed.
Involvement in pathological process in sciatica is possible not only one, and at once several roots of a spinal cord. On the level of defeat distinguish cervicothoracic sciatica (cervical and chest separately) and lumbosacral in the form of a lumbago or a lyumbalgiya and an ischialgia. Perhaps full damage of roots of all departments of a backbone.
Types of sciatica
Depending on in what place, roots are struck, sciatica is subdivided into the versions given below according to which the optimal variant of treatment of sciatica is chosen:
- upper cervical sciatica
- cervicobrachial sciatica
- chest sciatica
- lumbosacral sciatica
Physicians apply the term of sciatica (radiculopathy) to describe a complex of the symptoms appearing with a pressure upon the roots of a spinal cord (which are a bunch of the nerve fibrils proceeding from a spinal cord). Nevertheless, sciatica (radiculopathy) is not an independent disease, and only a complex of the symptoms including pain, muscle weakness and disturbances of sensitivity which originate in a backbone and go to extremities.
The human backbone is divided into five departments and sciatica (radiculopathy) most often meets in cervical chest and lumbar department. 31 couples of roots depart from a long spinal tyazh in quite narrow spinal canal of a backbone. Various reasons can reduce space where roots are located and to render on them a compression is hernia of a disk or a protrusion, a stenosis, a tumor, infectious process, an osteophyte. At all variability of symptoms the following symptoms most often meet:
- Pain (discomfort) from irradiation, stupid and periodic to continuous and wearisome with existence. Pain (as a protective factor in an organism) demonstrates that there is a damaging impact on nerve fibrils.
- Disturbance of sensitivity. It should be noted that a backbone rather complex structure. We unconsciously carry out a huge number of movements, and it is automatic. But in order that movements were harmonious, it is necessary that there was feedback of a brain from muscles and sensitive receptors. At a compression of roots there is a disturbance of carrying out impulses in both parties. Carrying out an impulse to muscles is respectively broken as sensitivity (numbness burning pricking), and.
- Weakness in muscles. Sometimes muscle weakness happens separately (at the isolated compression of motor neurons). Bilateral communication, as with a spinal cord, and head is necessary for normal functioning of muscles. At a root compression (interruption of receipt of impulses happens disk hernia, an osteophyte or still what - or), and the muscle ceases to work normally. At long disturbance of normal carrying out impulses there is an atrophy of muscles or even sluggish paresis.
Diagnosis of sciatica
Diagnostics is performed by the therapist, the neuropathologist. A round of researches which the doctor can appoint:
- Backbone X-ray analysis in direct and side projections, a basin with hip joints and other sites of a skeleton (according to indications).
- Computer tomography
- Magnetic resonance imaging
- Stsintigrafiya (scanning) of backbone bones
- Laboratory methods of a research (tests of blood, urine)
- Ultrasound examination of abdominal organs and small pelvis, retroperitoneal space
Causes of sciatica
Between vertebrae there are special openings through which the nerves departing from a spinal cord are passed. Sciatica arises at damage or inflammation of these nerves. As a rule (in 95 percent of cases), sciatica is manifestation of osteochondrosis. The remained cases are the started hernias of intervertebral disks and chronic spine injuries. To experience a sciatica attack, meanwhile, it is enough to make several awkward movements, to lift something heavy or to have a sleep in an inconvenient pose. Besides, the cause of sciatica to become frequent there are various infections.
Treatment of sciatica
Treatment of patients complex taking into account an etiology, pathogenesis, a stage and a course of the disease, use medicines of analgesic, dehydrating, antihistaminic, resorptional, anti-hypoxemic and sedative action. At the discogenic nature of sciatica for improvement of haemo circulation in a spinal canal appoint venotoniziruyushchy drugs (Venorutonum, Troxevasinum, Anavenolum, etc.). Apply vitamins, a balneoterapiya, physical therapy, manual therapy, remedial gymnastics, massage, orthopedic treatment. In an acute period at a sharp pain syndrome the bed rest, different types of novocainic blockade with additions of a hydrocortisone and B12 vitamin, intravenous drop administration of mix of Euphyllinum, Dimedrol, Relanium are shown. Painful zones irrigate with chloroethane or rub in them drugs of bee and snake poison, finalgon. At hernial protrusion of a disk the surgical treatment is shown. In sciatica of the infectious nature apply antibiotics of a broad spectrum of activity, corticosteroids, derivatives of salicylic acid.
After subsiding of acute pains appoint acupuncture, electrotreatment, massage, medical physical education, a balneoterapiya, mud cure. At a recurrent course and existence of the residual phenomena the sanatorium treatment using radonic or hydrosulphuric bathtubs and mud appliques is shown.
Treatment of sciatica folk remedies
1. Rye flour
Dough from rye flour is kneaded (without yeast). When dough becomes acid, to take the rag or a gauze put four times, to impose on a waist, and from above to impose dough 1 — 2 cm thick. To do daily for the night. There is enough prior to 10 procedures, and sciatica passes. Some add to dough of 30 drops of turpentine.
2. From unbleached linen to sew a belt with pockets. To put fruits of a horse chestnut in pockets and to carry in sciatica. It is considered that this means helps in sciatica.
3. Horse chestnut
To grind chestnut fruits together with a brown crust in flour. To slice black bread thin, to smear from above with camphor oil or creamy unsalted oil. From above on oil to strew flour of fruits of a chestnut. To impose on a sore point and to stick warm. Helps in sciatica.
4. Black radish
To clean, grate. On a cotton or linen rag to smear a small layer of radish and to cover with other rag. To impose on a sore point. From above to put a tracing-paper and to stick warm. To hold such compress until there is enough patience. At the same time the feeling of slow and deep warming up is created. Sometimes it is enough to make several times, and the disease recedes. Outwardly grated radish is used for grinding in sciatica, rheumatism, gout, in catarrhal diseases.
To pour in a saucer of a valerian (tincture), to obmochit in it a gauze or a rag, to impose on a sore point and to make a compress. To hold until there is enough patience.
To dissolve extract of a sage with water 1:5 or to make a strong profit of a sage grass. To put in the freezer. Then to rub with this ice a sore point.
To moisten a fresh leaf of a burdock in cold water, to rinse, a reverse side to impose on a sore point and to tie up. Good anesthetic. To dig burdock leaves with long shanks in the summer, to dry up them. It is enough to soak in lukewarm water in the winter and to apply as it is specified.
a) Mustard in the form of mustard plasters is applied as local irritating and a revulsive in catarrhal diseases, pleurisy, rheumatism, sciatica, neuritis.
b) Instead of mustard plasters it is possible to use mustard bathtubs: To stir 250 — 400 g of mustard powder, adding to it warm (not hot) water to consistence of thin gruel and to pound before emergence of a pungent caustic mustard smell. After that to pour out gruel in a bathtub (on 180 — 200 l of water) and it is good to mix. A bath is taken at a temperature of 35 — 36 °C of 5 — 6 minutes. After a bathtub to be washed 1 — 2 minutes under a warm shower and to muffle in a warm blanket. It is applied also in bronchitis, pneumonia.
9. For treatment of sciaticas to painful sites put a thick layer fresh leaves of a burdock, from above cover with compresseal paper and tie a warm scarf.
10. Compresses from juice and gruel of radish use in rheumatism, sciaticas and neuritis.
11. Alcohol 90-degree — 100 g, menthol — 2.5 g, anastezin — 1.5 g, novocaine — 1.5 g.
Sore points grease with this mix twice a day.
12. To cut out a cavity in a radish fruit root, to fill it with honey. In 4 hours, juice is ready. To pound sick body.
Diet in sciatica
Food of the people suffering from sciatica has to consist on two thirds of raw vegetables in the form of salads. It is possible to use cucumbers, tomatoes, cabbage, a garden radish. Carrots, spinach, a cauliflower are recommended to be steamed. To use many fruit, especially bananas. 4 times a day are better to eat: a breakfast – fruit and milk; a lunch – steamed vegetables and bread from coarse flour: an afternoon snack – fresh fruit or fruit juice; a dinner – fresh greens salad. Garlic is very useful: to eat every morning 2–3 zubochka. Relief is given by a lemon: juice of one lemon to salt and drink with water 2 times a day. It is necessary to avoid consumption of greasy, fried and spicy food, fat cottage cheese, confectionery, coffee and strong tea.
Prevention of sciaticas
Prevention of sciaticas includes the measures directed to maintenance of a correct posture, strengthening of muscles of a back. It is recommended to avoid excessive loads of a backbone. The correct posture and wakefulnesses minimizes during sleep tension which tests a spine column. It is not necessary to sleep on too rigid mattresses. Contrary to the occurring opinion they are capable to distort position of a backbone and to break its function. To avoid it and to keep a natural bend of a backbone it is recommended to pin up small pillows under a neck and a waist or to use a semifixed mattress which is capable to support natural position of a backbone.
Maintenance of a correct posture is very important also in daily work. Lifting objects from a floor, it is recommended to bend legs in knees, without inclining at this trunk. Thus, loading will move from a back to legs. It is recommended to avoid inconvenient poses (long sitting at a table with the hung head, sitting in front of the TV, having dropped a chin on a breast, etc.). Besides, in prevention of sciatica the gymnastics promoting strengthening of muscles of a back and also sports activities and hardening which increase resistance of an organism to physical activities and overcoolings has a big role.
